The home care plan will outline the following aspects of care:

• Schedule of Visits by the Care Professional
• Coordination of Care with the Medical Director or Physician
• Level of Assistance Required
• Mobility Support for Non-Ambulatory Patients
• Emergency Response, Fall Prevention and other Patient Education Resources
